Changes in Insurance

Please report any changes in your insurance information to our billing department as soon as possible. Reportable changes include change in insurance carrier, identification or group number, copay, benefit coverage, etc.

Please do not wait until your next appointment to report a change in your insurance information. It is critical that you report a change in insurance as soon as possible. Most treatments require prior authorization from your insurance company; an insurance change nullifies any previous prior authorizations obtained. Failure to report an insurance change may result in a delay in your treatment plan or may necessitate a change in your treatment plan. For these reasons, it is crucial to report a change in your insurance information to our billing department immediately.
